What Is OCT?

Your OCT scan is a cross-section of your retina — the same technology used in research hospitals, available at your annual eye exam. Understanding what it shows helps you have a more informed conversation with Dr. Payne about your long-term eye health.
Optical Coherence Tomography uses light waves to create high-resolution, cross-sectional images of the retina — the light-sensitive tissue at the back of the eye. Think of it as an ultrasound for the eye, but using light instead of sound, and with far greater resolution.
OCT produces images that show the individual layers of the retina in extraordinary detail — layers that are only a few microns thick. This allows us to detect structural changes in the retina and optic nerve that are invisible to standard examination, often years before they cause any symptoms or vision loss.
What Does an OCT Scan Feel Like?
The scan is completely non-invasive and painless. You simply rest your chin on a support, look at a target light, and the instrument scans your eye in seconds. No eye drops are required (though dilation may be recommended in some cases for a more complete view). The entire process takes about 5–10 minutes.
What Can OCT Detect?
OCT is particularly valuable for detecting and monitoring:
Glaucoma
Glaucoma damages the optic nerve, typically by thinning the retinal nerve fiber layer (RNFL) — the layer of nerve fibers that carry visual information from the retina to the brain. OCT can measure RNFL thickness with extraordinary precision and detect thinning that is too subtle to see with a standard examination. This allows us to identify glaucoma in its earliest stages, before any vision loss has occurred.
Macular Degeneration
Age-related macular degeneration (AMD) affects the macula — the central part of the retina responsible for sharp, detailed vision. OCT can detect the earliest signs of AMD, including drusen deposits and fluid accumulation beneath the retina, allowing for early intervention that can preserve central vision.
Diabetic Retinopathy
Diabetes can damage the blood vessels of the retina, leading to leakage, swelling, and eventually vision loss. OCT detects diabetic macular edema — swelling of the macula caused by leaking blood vessels — at a stage when treatment is most effective.
Macular Holes and Epiretinal Membranes
OCT is the definitive diagnostic tool for macular holes (small breaks in the macula) and epiretinal membranes (thin layers of scar tissue on the retinal surface), both of which can cause distorted or reduced central vision.
Retinal Detachment and Vitreous Traction
OCT can identify areas where the vitreous gel is pulling on the retina, a precursor to retinal tears and detachment.
Why OCT Matters: The Asymptomatic Window
The most important reason to include OCT in your regular eye care is that many serious eye diseases are completely asymptomatic in their early stages. Glaucoma, for example, typically causes no symptoms until significant peripheral vision has already been lost — and that loss is permanent. Macular degeneration may not cause noticeable central vision changes until the disease is moderately advanced.
OCT allows us to detect these diseases during the asymptomatic window — when treatment is most effective and vision loss can be prevented or minimized.
How Often Should You Have OCT?
For most patients, OCT is recommended as part of a comprehensive eye exam every 1–2 years. More frequent monitoring is recommended for patients with:
- A family history of glaucoma or macular degeneration
- Diabetes or other systemic conditions affecting the eyes
- Elevated intraocular pressure
- Suspicious optic nerve appearance on examination
- Previous retinal conditions
