What Is Demodex?

Demodex are microscopic mites that naturally live on human skin, but when they overpopulate the eyelash follicles and meibomian glands, they cause a chronic inflammatory condition called Demodex blepharitis. It is estimated that Demodex is present in over 50% of the population — and in nearly 80% of patients over age 60.

How We Detect It

Dr. Payne performs a detailed slit-lamp examination of the eyelid margins and lashes, looking for the characteristic "collarette" deposits — cylindrical dandruff-like debris at the base of the lashes that is the hallmark sign of Demodex infestation. Meibomian gland imaging and tear film analysis help assess the downstream impact on dry eye.

How We Treat It

Treatment begins with in-office NuLids and Rinsada eyelid debridement to physically remove mites and biofilm from the lid margins. IPL therapy is highly effective at reducing Demodex populations through targeted light energy. We also prescribe Xdemvy (lotilaner ophthalmic solution 0.25%) — the first FDA-approved prescription treatment specifically for Demodex blepharitis — along with a personalized at-home lid hygiene regimen using tea tree oil-based cleansers.
